This is your own invention, so there is no single right answer. But the form has six headings, and each one asks for something particular.

Before you write, copy the way Hekko is built: it has a circle, two teams of a fixed size, one clear way to get a player out, and three clear ways for the game to end. Give your game all four of those things and the rules will be easy to write.

Model answer:
Game: Ring Rescue
Time: 10 minutes
Number of players: 8 — 4 in each team
Material needed: Chalk, one soft ball, four small stones
Rules:
1. Draw a big circle with chalk. Put the four stones in the middle.
2. Team A stands inside the circle and guards the stones.
3. Team B stands outside and rolls the ball to knock a stone over.
4. Team B gets one point for every stone knocked over.
5. After five minutes the teams change places.
6. The team with more points wins the game.
Foul:
1. No player may throw the ball above the knee.
2. No player may pull or push another player.
3. A player who commits a foul must sit out for one minute.
Why every game needs a “foul” list: Rules tell you how to win. Fouls tell you what is not allowed while you try. Without them a strong player could simply push everyone away, and the game would stop being fun.
Check your work: Read your rules to a friend who has never heard of your game. If your friend can play it after one reading, your rules are clear enough.
